I think Duncan didn't recognize Mal due to how much he changed. At 14, he'd be around 5'8, but at 17, he's 6'6. His hair is also significantly longer, and his skin would be tanner.
Do I believe Mal was "running juvie" at 12-16? No 馃拃. He was definitely someone who fought to win, but seeing him on TDAS, he definitely didn't have the socialization skills to command ppl.
